Birch Bark (Betula papyrifera): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does birch bark work to heal wounds?

Birch bark contains betulin and betulinic acid, which are the primary compounds responsible for its wound healing properties. These triterpenes demonstrate anti-inflammatory and antimicrobial activities that help reduce infection risks and accelerate tissue repair (Evidence Level C). Additionally, the tannins in the bark provide an astringent effect, helping to tighten skin and reduce secretion from wounds. This mechanism supports its traditional use by Indigenous peoples for treating burns, cuts, and skin ulcers.

Is birch bark safe to take during pregnancy?

No, birch bark is considered unsafe during pregnancy and should be avoided (Evidence Level C). The bark contains methyl salicylate, a compound chemically similar to aspirin, which may pose risks for fetal development and pregnancy outcomes. Large doses can potentially cause complications. Women who are breastfeeding should also avoid birch bark due to a lack of safety data. It is crucial to consult a healthcare provider before using any herbal remedies while pregnant.

What is the recommended dosage for birch bark tea?

The standard adult dosage for birch bark tea is 1 to 2 cups daily (Evidence Level: Traditional). To prepare, steep approximately 1 to 2 grams of dried birch bark in boiling water for 10 to 15 minutes. For topical applications, a stronger infusion can be prepared and applied to the skin using a clean cloth. Dosage should not be exceeded without medical supervision, especially for individuals with kidney concerns.

Does birch bark interact with medications like blood thinners?

Yes, birch bark may interact with anticoagulant and antiplatelet medications (Evidence Level D). Because the bark contains methyl salicylate, which has blood-thinning properties similar to aspirin, taking it alongside drugs like warfarin or aspirin may increase the risk of bleeding. Additionally, due to its potential to irritate the kidneys, it may interact with diuretics or medications affecting renal function. Always consult a physician before combining herbal supplements with prescription drugs.

What are the side effects of using birch bark internally?

While generally well-tolerated in small amounts, birch bark can cause side effects in some individuals. The most common risks include allergic skin reactions or respiratory distress in those sensitive to birch pollen. Internal use of large doses may lead to kidney irritation due to the presence of tannins and methyl salicylate (Evidence Level C). People with pre-existing kidney disease should avoid internal use. Discontinue use immediately if urinary changes or skin rashes occur.

Résumé

Birch bark (Betula papyrifera and other species) contains triterpenoids like betulin, which are used in topical treatments for wound healing and exhibit various pharmacological activities [PMID:37658982, PMID:35684557, PMID:37509141].

Contexte

Birch bark is derived from species such as Betula papyrifera (paper birch) and Betula platyphylla [PMID:31580010, PMID:41166909]. It is a source of lupan-series triterpenoids and has been used historically to produce birch bark tar, a water-resistant and biocidal material [PMID:35684557, PMID:32190727].

Usages traditionnels

Historically, birch bark tar has been used as a multipurpose adhesive, sealant, and in medicine [PMID:32190727]. In Chinese medicine, Betula platyphylla Suk is used to clear heat, detoxify, relieve cough, and resolve phlegm [PMID:41166909].

Composés actifs

The primary active compounds are triterpenoids, most notably betulin and its derivative betulinic acid [PMID:35684557, PMID:23848199]. Other identified components in B. papyrifera include coumaroylquinic acid, betuloside pentoside A, and a diarylheptanoid hexoside [PMID:22012323].

Mécanisme d'action

Betulin and birch bark extracts promote wound healing by increasing the migration of primary human keratinocytes, accelerating wound closure, and promoting keratinocyte differentiation [PMID:31168940]. Betulin modulates inflammation by blocking proinflammatory cytokines via NF-κB and MAPKs pathways [PMID:37509141, PMID:34961132]. In cancer, betulin may inhibit tumor angiogenesis by inhibiting the PAX2/VEGF-A/VEGR2 signaling pathway [PMID:41166909] and induce apoptosis [PMID:37509141]. Betulinic acid may modulate the PPAR-γ/NF-κB signaling pathway to reduce intestinal inflammation [PMID:40647158].

Preuves cliniques

Preuve BEpidermolysis Bullosa (EB)

A topical gel (Oleogel-S10) consisting of 10% dry birch bark extract significantly increased the proportion of patients with first complete target wound closure within 45 days compared to control gel [PMID:37658982, PMID:42205007].

Preuve CDry Skin

Topical application of Betula alba (birch bark extract) increased skin hydration and reduced transepidermal water loss in humans in vivo [PMID:21709432].

Preuve CActinic Keratosis

Birch bark is listed as a herbal biomedicine tested for the treatment of actinic keratosis [PMID:32046246].

Sécurité et effets indésirables

Birch bark extract (Oleogel-S10) was reported to be generally well tolerated in patients with epidermolysis bullosa [PMID:37658982].

Synthèse des preuves

Evidence is strongest for the topical use of birch bark extract (Oleogel-S10) in treating wounds associated with epidermolysis bullosa, supported by Phase III clinical trial data. Other pharmacological claims (anticancer, anti-inflammatory) are primarily based on in vitro, in vivo animal models, or review articles.
