Buchu (Agathosma betulina) is a South African herb traditionally used by the Khoikhoi for urinary infections and as a body perfume. Modern research indicates diuretic, antimicrobial, and anti-inflammatory activities attributed to diosphenol, pulegone, and flavonoids. It is used for UTIs and kidney support, but safety concerns exist due to pulegone content.
Diosphenol and volatile oils exhibit antimicrobial activity against common uropathogens such as Escherichia coli. Flavonoids provide anti-inflammatory effects via inhibition of cyclooxygenase (COX) and lipoxygenase pathways. Pulegone undergoes hepatic CYP450 metabolism to form toxic metabolites that can cause renal and hepatic irritation. The diuretic effect is thought to involve increased renal blood flow and inhibition of sodium reabsorption in the proximal tubules.
