How does Commiphora wightii work to lower cholesterol?
Commiphora wightii, commonly known as guggul, works primarily through its active compounds, guggulsterones Z and E. These compounds act as antagonists to the farnesoid X receptor (FXR), a key regulator of cholesterol and bile acid metabolism. By blocking this receptor, guggul helps increase the conversion of cholesterol into bile acids and promotes its excretion from the body. While this mechanism is well-documented in pharmacological research, clinical evidence (Level C) regarding its efficacy in humans remains mixed compared to conventional statin therapy.
Does guggul interact with thyroid medication?
Yes, Commiphora wightii has significant interactions with thyroid medications. Guggul may stimulate thyroid function, potentially altering thyroid hormone levels. If taken alongside thyroid hormone replacement therapy like levothyroxine, it may interfere with the intended dosage and efficacy, leading to symptoms of hyperthyroidism or inconsistent thyroid panel results. Patients on thyroid medication should avoid this herb or be monitored closely by a physician. This interaction is supported by pharmacological data and theoretical evidence (Level C).
What is the recommended dosage for Commiphora wightii extract?
The standard adult dosage for Commiphora wightii is typically 500 mg to 1 gram of standardized guggulipid extract taken two to three times daily. This extract should be standardized to contain approximately 2.5% to 5% guggulsterones to ensure therapeutic potency. Because the raw resin can cause digestive upset, standardized extracts are preferred over crude gum preparations. Clinical studies investigating cholesterol and arthritis have utilized these specific dosage ranges, though individual needs may vary based on the specific health concern.
Is Commiphora wightii safe to take during pregnancy?
No, Commiphora wightii is considered unsafe during pregnancy and breastfeeding. The herb has been traditionally used as an emmenagogue, meaning it can stimulate blood flow in the pelvic area and uterus, which may induce uterine contractions and potentially lead to miscarriage or premature labor. Due to these safety concerns and a lack of teratogenicity data, pregnant women should strictly avoid guggul supplements. This contraindication is supported by both traditional use knowledge and safety classifications.
Is guggul effective for treating arthritis?
Guggul has been used in Ayurvedic medicine for centuries to treat inflammatory joint conditions like arthritis. Preliminary research (Level C) suggests that the guggulsterones in the resin possess anti-inflammatory properties that may help reduce joint pain and swelling, comparable to some non-steroidal anti-inflammatory drugs (NSAIDs). However, clinical trial data is limited and sometimes conflicting regarding its efficacy compared to standard treatments. While some patients report relief, individuals with arthritis should consult a rheumatologist before substituting conventional care with herbal remedies.

Résumé
Commiphora wightii, commonly known as guggul, is a tree whose oleo-gum resin is used in traditional Ayurvedic, Siddha, and Unani medicine for treating obesity, inflammation, and lipid disorders [PMID:29746995, PMID:26587309].
Contexte
Commiphora wightii (syn. Commiphora mukul) is a plant native to the dry regions of India (specifically Gujarat, Rajasthan, and Madhya Pradesh) and bordering regions of Pakistan [PMID:26587309, PMID:23716870]. The medicinal part is an oleo-gum resin tapped from incisions made in the bark [PMID:26587309].
Usages traditionnels
Traditionally used in Ayurveda for centuries to treat obesity, liver disorders, internal tumors, malignant sores, ulcers, urinary complaints, intestinal worms, leucoderma, sinus, edema, sudden paralytic seizures, inflammation, gout, and rheumatism [PMID:26561056, PMID:26587309, PMID:27771932].
Composés actifs
The primary bioactive constituents are the plant sterol isomers E- and Z-guggulsterone [PMID:12626688, PMID:32110820]. Other identified metabolites include amino acids, fatty acids, organic acids, phenolic acids, pregnane-derivatives, steroids, sterols, sugars, sugar alcohols, terpenoids, tocopherol, quinic acid, and myo-inositol [PMID:25561401, PMID:26587309].
Mécanisme d'action
Guggulsterones act as antagonist ligands for the farnesoid X receptor (FXR), which regulates bile acid and cholesterol homeostasis [PMID:12626688, PMID:27771932]. They also modulate gene expression by regulating transcription factors such as NF-kappaB, STAT-3, and C/EBPalpha, and downregulating anti-apoptotic proteins (e.g., Bcl-2, survivin) and cell cycle proteins (cyclin D1, c-Myc) [PMID:26561056, PMID:27771932, PMID:19189646].
Preuves cliniques
Preuve AHyperlipidemia
Systematic review and other studies indicate significant decreases in total cholesterol and LDL cholesterol [PMID:20858178, PMID:16338199], although some data in Western populations show modest or inconsistent results [PMID:19114224, PMID:37300664].
Preuve CCancer/Tumors
Guggulsterone inhibits the growth of various tumor cells, induces apoptosis, and suppresses osteosarcoma progression by inhibiting glycolysis via the MAPK signaling pathway [PMID:26561056, PMID:40517691].
Preuve CInflammation/Arthritis
Used for treating inflammation, rheumatism, and joint disorders [PMID:32110820, PMID:26587309].
Synthèse des preuves
The evidence for lipid-lowering effects is supported by systematic reviews and clinical trials, though results vary by population. Anti-cancer and anti-inflammatory claims are largely supported by in vitro, animal models, and review articles.
