Prunella vulgaris (Selfheal) is a widely distributed herb used traditionally across Europe, Asia, and North America for wound healing, sore throat, and viral infections. Its primary active constituents include rosmarinic acid, ursolic acid, oleanolic acid, and hyperoside, which contribute to its anti-inflammatory, antiviral, and hypotensive properties. Modern evidence (Level C) supports its use as an adjunctive therapy for mild hypertension, herpes simplex, and minor wounds.
Rosmarinic acid inhibits 5-lipoxygenase and cyclooxygenase-2, reducing leukotriene and prostaglandin synthesis, thereby exerting anti-inflammatory effects. Ursolic and oleanolic acids suppress NF-κB activation and downregulate pro-inflammatory cytokines (TNF-α, IL-6). Hyperoside and other flavonoids demonstrate antiviral activity by inhibiting viral entry and replication, particularly against herpes simplex virus (HSV) and influenza. The hypotensive effect is attributed to ACE inhibition and calcium channel blockade by triterpenoids and phenolic acids.
