Hedyotis Herb (Hedyotis diffusa): Benefits, Dosage, Safety & Evidence

Frequently asked questions

How does Hedyotis diffusa work against cancer cells?

Hedyotis diffusa contains active compounds like ursolic acid and oleanolic acid that demonstrate antitumor activity in preclinical research. These compounds appear to induce apoptosis (programmed cell death) and inhibit tumor cell proliferation. In Traditional Chinese Medicine (TCM), the mechanism is understood as clearing heat and fire toxins, addressing the inflammatory patterns often associated with malignancy. Current evidence (Level C) relies primarily on in vitro and animal studies rather than large-scale human clinical trials.

Can I take Hedyotis diffusa while undergoing chemotherapy?

Patients undergoing chemotherapy should exercise caution and consult their oncologist before using Hedyotis diffusa. While it is frequently combined with conventional treatments in TCM protocols, specific drug interaction data remains limited. Theoretically, its diuretic action may alter the excretion rates of certain drugs, and its effects on blood clotting pathways could interact with anticoagulants. Professional guidance is essential to ensure the herb does not interfere with treatment efficacy or safety.

What is the standard dosage for Hedyotis diffusa?

The standard adult dosage for Hedyotis diffusa varies by preparation. For a dried herb decoction, practitioners typically prescribe 15 to 30 grams daily. For standardized extracts, the common dosage is 500 to 1000 mg per day. Because this herb has a cold energetic nature, it is crucial to follow a qualified practitioner's guidance to determine the appropriate strength and duration, as higher doses may cause gastrointestinal upset in some individuals.

Is Hedyotis diffusa safe to take during pregnancy?

No, Hedyotis diffusa is considered unsafe during pregnancy and is strictly contraindicated. In Traditional Chinese Medicine, the herb invigorates blood and dispels stasis, properties that may stimulate uterine contractions and increase the risk of miscarriage. Breastfeeding women should also avoid this herb. Additionally, individuals with a 'Cold' constitution—characterized by cold hands, feet, and loose stools—should use it with caution to prevent exacerbating their condition.

What are the main uses of Hedyotis diffusa in traditional medicine?

In Traditional Chinese Medicine (TCM), Hedyotis diffusa is primarily used to clear heat, resolve toxicity, and dispel blood stasis. It is one of the most common herbs prescribed in TCM cancer support formulas. Practitioners also utilize it for urinary tract infections and inflammatory conditions associated with damp-heat patterns. Its cooling properties make it effective for 'hot' conditions, though it is traditionally avoided in patients with a cold constitution.

Summary

Hedyotis diffusa is a traditional Chinese medicine (TCM) recognized for its anti-tumor, anti-inflammatory, and immunomodulatory properties [PMID:41084455, 27248992].

Background

Hedyotis diffusa Willd (also known as Baihuasheshecao) is a medicinal herb used in TCM, often as part of a classic herb pair with Scutellaria barbata for clearing heat and detoxifying [PMID:41084455, 42261445].

Traditional uses

It has been widely utilized in folk medicine and TCM clinical practice for treating malignant tumors, as well as for its antibacterial and anti-inflammatory properties [PMID:41084455, 29189741].

Active compounds

The herb contains a diverse range of phytochemicals, including iridoids (e.g., asperulosidic acid), flavonoids (e.g., quercetin, kaempferol), anthraquinones, phenolics, sterols (e.g., ursolic acid, beta-sitosterol), volatile oils, and polysaccharides [PMID:27248992, 34321173, 35545835, 34975504, 11170670].

Mechanism of action

Mechanisms include the induction of ferroptosis via the Bax/Bcl2/VDAC2/3 axis [PMID:35853302], suppression of Src/FAK signaling to inhibit pulmonary metastasis [PMID:41380412], and modulation of the PI3K/Akt/NF-κB pathway [PMID:39839649]. It also acts by silencing CYP19A1/aromatase [PMID:35545835] and activating the IL-2/IL-2R pathway to enhance T cell infiltration [PMID:39954887].

Clinical evidence

Evidence DLung Adenocarcinoma

Hedyotis diffusa injection induces ferroptosis and exhibits anti-tumor activity in vitro and in nude mouse models [PMID:35853302]

Evidence DOvarian Cancer

Combined with Scutellaria barbata and cisplatin, it synergistically inhibits cell viability, colony formation, and migration via the NRF2-FTH1 pathway [PMID:39702302]

Evidence DColorectal Cancer

Polysaccharides from the herb enhance the effectiveness of PD-1 and CTLA-4 inhibitors by increasing CD4+ and CD8+ T cell infiltration [PMID:39954887]; it is also noted for overcoming multidrug resistance [PMID:32740824]

Evidence CHepatocellular Carcinoma

EV-like particles (HDW-EVLPs) suppress Huh-7 cell proliferation and promote apoptosis [PMID:40744176]; combined with Scutellaria barbata, it modulates apoptosis and immune responses [PMID:42261445]

Evidence DGastric Cancer

Ursolic acid enriched in the herb silences CYP19A1 to suppress growth [PMID:35545835]; Fufang She-She-Cao formula is used to treat precancerous lesions [PMID:40185259]

Evidence D

Drug interactions

Pre-treatment with Hedyotis diffusa extract dose-dependently decreased the area under curves (AUCs) and relative bioavailability of tamoxifen by 31-68% in rat models [PMID:34839255].

Evidence summary

The evidence consists primarily of in vitro studies and animal models (Level D) and a few systematic reviews (Level A/C). While promising anti-tumor and anti-inflammatory activities are demonstrated, high-level human clinical trial data is lacking in the provided abstracts.
