Pandanus Root (Pandanus tectorius): Benefits, Dosage, Safety & Evidence

Frequently asked questions

How does Pandanus root work as a diuretic?

Pandanus root exerts its diuretic effects through several bioactive compounds. The flavonoids and tannins present in the root are believed to increase renal blood flow and promote urine output. Beta-sitosterol, a key active compound, may help reduce inflammation in the urinary tract, while essential oils contribute additional therapeutic properties. Evidence level C supports these traditional diuretic uses, though more rigorous clinical studies are needed to fully elucidate the mechanism of action.

Does Pandanus root interact with any medications?

Pandanus root may interact with diuretic medications such as furosemide or hydrochlorothiazide, potentially enhancing their effects and increasing the risk of dehydration and electrolyte imbalances. Due to its diuretic properties, it may also affect how the body eliminates certain drugs, altering their effectiveness. Those taking lithium should exercise particular caution, as increased urination can raise lithium levels to potentially dangerous concentrations. Always consult a healthcare provider before combining herbal remedies with prescription medications.

What is the recommended dosage for Pandanus root tea?

The traditional adult dosage for Pandanus root is 15-30 grams of dried root prepared as a tea, taken daily. To prepare, steep the dried root in hot water for 10-15 minutes before straining. This dosage is based on traditional use across Pacific Island communities and is supported by Evidence level C. Dosage should be adjusted based on individual response and under guidance from a qualified healthcare practitioner. Do not exceed recommended amounts without professional advice.

Is Pandanus root safe to take during pregnancy?

No, Pandanus root is considered unsafe during pregnancy and should be avoided. Traditional use and available evidence indicate that the diuretic properties of this herb may stimulate uterine contractions or affect fluid balance in ways that could potentially harm fetal development. Pregnant women should avoid all preparations containing Pandanus root. Women who are breastfeeding should also consult a healthcare provider before use, as safety during lactation has not been established. This contraindication is well-documented in traditional Pacific medicine.

What is Pandanus root used for medicinally?

Pandanus root has been used traditionally across Pacific Island communities for treating urinary conditions, including urinary tract infections and difficulty urinating. It is also employed for reducing edema (fluid retention), managing fever, and alleviating symptoms of rheumatism. The anti-inflammatory properties attributed to beta-sitosterol and flavonoids may explain its effectiveness for inflammatory conditions. Evidence level C supports these traditional applications, meaning they are based on limited scientific study but extensive traditional knowledge spanning generations.

Traditional uses

Pandanus tectorius has been traditionally used in Ayurvedic medicine to treat certain diseases [PMID:32154156].

Active compounds

Compounds identified in P. tectorius include caffeoylquinic acids (CQAs) [PMID:24629909, 23613974], tangeretin, ethyl caffeate [PMID:33132720], and various aldehydes such as p-hydroxybenzaldehyde, syringaldehyde, and vanillin [PMID:25674660]. Other constituents include a new dihydrofurocoumarin (pandanusin A), furanocoumarins, coumarins, lignans, neolignans, flavonoids, and phenolics [PMID:27228410]. Leaf extracts contain stigmasterol [PMID:40231614] and a new lignan, (8S, 8'S)-2,2',3,3'-tetramethoxy-4'-hydroxy-epoxylignan-4-O-β-D-glucoside [PMID:34963388].

Mechanism of action

P. tectorius fruit extract may exert anti-hyperlipidaemic effects by modulating gut microbiota [PMID:30836215], upregulating scavenger receptor-B1 (SR-B1) gene expression, and downregulating 3-hydroxy-3-methylglutaryl coenzyme A reductase (HMGCR) [PMID:32154156, 33132720]. It may also stimulate phosphorylation of AMP-activated protein kinase (AMPK) and Akt substrate of 160 kDa (AS160) [PMID:24629909] and activate the Nrf2/Keap1 signaling pathway to reduce oxidative stress [PMID:39887023]. Leaf extracts may inhibit quorum sensing by binding to the cviR master regulator protein [PMID:40231614] and modulate COX-2 expression for anti-inflammatory effects [PMID:32089985].

Clinical evidence

Evidence DHyperlipidemia/Hypercholesterolemia

Fruit extracts decreased serum total cholesterol, triglycerides, and LDL-c in hamsters [PMID:23613974] and ameliorated high-fat diet-induced hyperlipidaemia in mice [PMID:30836215].

Evidence DDiabetes/Hyperglycemia

CQA-rich fruit extract significantly decreased body weight, fasting glucose levels, and alleviated hyperinsulinism in diabetic db/db mice [PMID:24629909].

Evidence DInflammation

Aqueous leaf extract (500 mg/kg) significantly reduced carrageenan-induced rat paw edema and reduced COX-2 expression [PMID:32089985].

Evidence DDiabetic Peripheral Neuropathy

Fruit extracts reduced pain and prevented nerve damage in DPN rats by activating the Nrf2/Keap1 pathway [PMID:39887023].

Evidence DCancer (In Vitro)

A new lignan from leaves exhibited moderate cytotoxicity against HeLa cells (IC50 = 19.30 ± 4.46 μM) [PMID:34963388].

Safety & adverse effects

P. tectorius fruit was found to be noncytotoxic against the HepG2 cell line [PMID:32154156]. Leaf extract was confirmed as non-toxic in assays regarding anti-quorum sensing [PMID:40231614]. However, excessive saponin enrichment in fruit extracts coincided with cytotoxicity [PMID:41497563].

Evidence summary

The current evidence is based primarily on in vitro studies and animal models (mice, rats, hamsters, and aquatic species), indicating potential anti-hyperlipidemic, anti-diabetic, and anti-inflammatory properties. There are no human clinical trials provided.
