Spirulina's primary active compound, phycocyanin, provides potent antioxidant and anti-inflammatory effects by scavenging free radicals and inhibiting inflammatory pathways. Its gamma-linolenic acid supports anti-inflammatory prostaglandin production, while beta-carotene and vitamin B12 contribute to cellular protection and energy metabolism. Spirulina also modulates immune function by enhancing macrophage activity and promoting antibody production. These combined mechanisms support its use for immune support, cardiovascular health, and detoxification. Evidence Level: C.
Can I take spirulina with blood thinners or other medications?
Spirulina may interact with anticoagulant and antiplatelet medications, potentially increasing bleeding risk due to effects on platelet aggregation. Those taking warfarin, aspirin, or similar medications should consult a healthcare provider before use. Spirulina may also stimulate immune function, potentially conflicting with immunosuppressive drugs prescribed for autoimmune conditions or transplant patients. Evidence Level: C. Always disclose all supplements to your prescribing physician to avoid adverse interactions.
What is the recommended daily dosage of spirulina?
The typical adult dosage of spirulina ranges from 1 to 10 grams daily, most commonly taken as 3 to 5 grams in divided doses. Powder forms can be mixed into smoothies or water, while tablets usually provide 500mg each. Higher doses may be used for specific conditions under professional guidance. Evidence Level: C. Start with lower doses to assess tolerance, as initial use may cause mild gastrointestinal upset that typically resolves with continued use.
Is spirulina safe to take during pregnancy and breastfeeding?
Spirulina is generally considered safe during pregnancy and breastfeeding when sourced from reputable suppliers. Its nutrient density, including complete protein, iron, and B vitamins, may offer nutritional support for expectant mothers. However, contamination with microcystins or heavy metals from poor-quality sources poses significant risks to fetal development. Pregnant women should only use third-party tested products certified free of contaminants and consult their healthcare provider before starting supplementation. Evidence Level: Traditional use with limited clinical data.
Who should not take spirulina supplements?
Individuals with autoimmune diseases such as lupus, rheumatoid arthritis, or multiple sclerosis should avoid spirulina, as its immune-stimulating properties may exacerbate symptoms. Those with phenylketonuria (PKU) cannot properly metabolize phenylalanine present in spirulina's protein content. Additionally, anyone with known allergies to blue-green algae or seafood should exercise caution. Evidence Level: C. Always consult a qualified healthcare provider before starting supplementation if you have underlying health conditions.

Summary
Spirulina platensis is a blue-green microalga used as a nutritional supplement with reported antioxidant, anti-inflammatory, and neuroprotective properties [PMID:36453480, PMID:33612469].
Background
Spirulina platensis (also known as Arthrospira platensis) is a photosynthetic, spiral-shaped cyanobacterium from the class Cyanophyceae found in fresh water [PMID:30000909, PMID:36453480, PMID:38110108].
Traditional uses
It has been applied for over 400 years and is traditionally used as a food supplement and medical ingredient [PMID:36453480, PMID:36632963].
Active compounds
Key components include proteins, B vitamins (B1, B2), vitamin E, chlorophyll, beta-carotene, iron, phycocyanin, polyphenols, carotenoids, and gamma-linolenic fatty acid [PMID:30000909, PMID:36798131, PMID:15049159]. It also contains polysaccharides (SPPs) and very-long-chain polyunsaturated fatty acids (VLCPUFAs) such as precursors to EPA and DHA [PMID:36632963, PMID:35023103].
Mechanism of action
Spirulina exhibits antioxidant activity by increasing transcript levels of NRF-2 and Keap1 while decreasing gene expressions of Akt and eNOS [PMID:36432871]. Its polysaccharides may regulate gut microbiota properties [PMID:36632963]. Additionally, its polyphenolic contents may inhibit bacterial motility, invasion, biofilm formation, and quorum sensing by weakening bacterial cell walls [PMID:36798131].
Clinical evidence
Evidence BRebound dyspeptic symptoms after PPI discontinuation
A phase 2 placebo-controlled trial assessed whether Spirulina platensis can minimize rebound symptoms through anti-inflammatory and analgesic properties [PMID:38401078]
Evidence CGeneral Pharmacological Effects
Review of literature attributes antioxidative, anti-inflammatory, anti-neoplastic, hypolipidemic, antiviral, immunomodulatory, antimicrobial, anti-atherogenic, anti-diabetic, and radio-protective functions to Spirulina [PMID:36453480]
Evidence DOxidative Stress (HepG2 cells)
Spirulina platensis demonstrated a protective role against antiretroviral drug-induced oxidative stress in human liver cells [PMID:36432871]
Evidence DSchizophrenia (Animal Model)
Spirulina platensis showed neuroprotective effects in ameliorating psychosis-like symptoms in a dizocilpine-induced rat model [PMID:33612469]
Safety & adverse effects
Generally well tolerated when grown under controlled conditions [PMID:30000909]. Minor adverse effects include diarrhea, bloating, upset stomach, flatulence, edema, headache, muscle pain, facial flushing, and sweating [PMID:30000909]. Products grown in uncontrolled conditions may contain heavy metals or be contaminated with Microcystis aeruginosa, which produces hepatotoxins called microcystins [PMID:30000909].
Pregnancy & lactation
No specific lactation-related uses are known; no data exist on safety, efficacy, or excretion into breastmilk for nursing mothers or infants, although one case of green breastmilk discoloration was reported [PMID:30000909].
Evidence summary
Evidence ranges from high-level clinical trials for specific gastrointestinal symptoms to preclinical animal and in vitro studies supporting broad antioxidant and neuroprotective claims. Much of the available data is based on reviews or animal models.
