Zingiber montanum (Zingiber montanum): Benefits, Dosage, Safety & Evidence

Frequently asked questions

How does Zingiber montanum work as an anti-inflammatory?

Zingiber montanum contains bioactive compounds including camphor, cineole, terpinen-4-ol, alpha-pinene, and phenylbutadienes that contribute to its anti-inflammatory effects. These compounds work by inhibiting pro-inflammatory mediators and reducing oxidative stress in affected tissues. The phenylbutadienes in particular have demonstrated anti-inflammatory activity in laboratory studies. The herb's mechanism is similar to other medicinal gingers used traditionally for pain and inflammation relief. Evidence level: C (preliminary clinical evidence).

Does Zingiber montanum interact with any medications?

Limited clinical data exists on specific drug interactions for Zingiber montanum. Due to its anti-inflammatory properties and potential effects on blood clotting, it may theoretically interact with anticoagulant or antiplatelet medications such as warfarin or aspirin. Caution is also advised when combining with diabetes medications or blood pressure drugs, as it may alter their effects. Always consult a healthcare provider before using this herb alongside prescription medications. Evidence level: C.

What is the recommended dosage for Zingiber montanum?

For adults, the typical internal dosage of Zingiber montanum rhizome is 3-6 grams daily. For topical applications, the herb can be applied as needed to affected areas for muscle pain or inflammation. Preparations vary depending on the condition being treated and the form used, whether fresh, dried, or extracted. Always follow product label instructions or consult a qualified herbalist for personalized dosing recommendations. Evidence level: Traditional use.

Is Zingiber montanum safe to use during pregnancy?

Zingiber montanum is considered unsafe during pregnancy, particularly at large internal doses. The herb contains compounds such as camphor that may stimulate uterine contractions or cause other adverse pregnancy outcomes. Women who are pregnant or breastfeeding should avoid internal use of this herb entirely. Topical use should also be avoided unless specifically recommended by a qualified healthcare provider familiar with the individual's medical history. Evidence level: C.

What is Zingiber montanum used for in traditional medicine?

Zingiber montanum has been traditionally used in Southeast Asian medicine for treating muscle pain, rheumatism, and digestive disorders. It is valued for its anti-inflammatory and analgesic properties, similar to Cassumunar ginger. The rhizome is commonly applied topically for joint and muscle discomfort, or taken internally for digestive complaints. Traditional healers have used this highland ginger extensively throughout Southeast Asia for centuries. Evidence level: C.

What are the side effects of Zingiber montanum?

Topical application of Zingiber montanum may cause skin irritation in some individuals, particularly those with sensitive skin or allergies to ginger species. Internal use at recommended doses is generally well-tolerated, though excessive doses may cause gastrointestinal discomfort. Due to its camphor content, large internal doses should be avoided as they may cause toxicity. Discontinue use if skin reactions occur and seek medical attention if symptoms persist. Evidence level: C.

Summary

Zingiber montanum (also known as Phlai or Bangle) is a medicinal herb from the Zingiberaceae family used primarily for its anti-inflammatory, analgesic, and antimicrobial properties [PMID:33921835, 32735595].

Background

Zingiber montanum (Z. montanum), also referred to as Zingiber cassumunar Roxb., is a medicinal and ornamental herb found in tropical regions, particularly Southeast Asia [PMID:32735595, 40380132]. It is known as "Plai" in Thailand, "Bangle" in Indonesia, and "Bulei" in China [PMID:33921835].

Traditional uses

Traditionally, the rhizomes have been used to treat inflammation, pain, respiratory problems [PMID:33921835], bruises, sprains, and musculoskeletal pain [PMID:29154071]. It has also been used as an anti-asthmatic drug in Thai traditional medicine [PMID:25543044, 25840633] and to ameliorate peptic ulcers in northern Bangladesh [PMID:22366683].

Active compounds

Key constituents include phenylbutenoids, curcuminoids, and essential oils [PMID:33921835]. Specific identified compounds include sabinene, terpinen-4-ol, gamma-terpinene [PMID:32019210, 37202876], curcumin, beta-sitosterol, and lauric acid [PMID:40141705], as well as (E)-1-(3,4-dimethoxyphenyl)but-1-ene, (E)-1-(3,4-dimethoxyphenyl)butadiene, and zerumbone [PMID:1804548].

Mechanism of action

The herb demonstrates anti-inflammatory effects by inhibiting the phosphorylation of ERK1/2, p38, and JNK [PMID:23278498], and suppressing PMA-induced MUC2 and MUC5AC expression via ERK inhibition [PMID:25543044]. It also inhibits MMP-9 direct activation and expression [PMID:25840633] and targets NF-kappaB, Akt/MAPK, and caspase-3 pathways [PMID:40380132].

Clinical evidence

Evidence BAllergic Rhinitis

Phlai capsules (100 mg and 200 mg) improved reflective total five symptom scores (rT5SS) compared to placebo [PMID:37302097]. 200 mg capsules significantly reduced nasal IL-5, IL-13, and eosinophil counts [PMID:37302093].

Evidence AMusculoskeletal Pain / Sprains

14% Plai cream showed a strong trend of benefit in pain reduction for muscle pain and ankle sprains [PMID:29154071]. 14% Plai cream substantially reduced delayed onset muscle soreness (DOMS) by 82% compared to placebo [PMID:26988432].

Evidence BPainful Diabetic Neuropathy

Topical application of 15% w/w Plai balm significantly reduced mean pain scores compared to placebo at weeks 2 and 4 [PMID:39220605].

Evidence BMyofascial Pain Syndrome

Phlai spray cool formula (PSCF) showed comparable reductions in pain intensity and improvements in neck disability index and cervical range of motion compared to diclofenac spray [PMID:40141705].

Evidence BCognition

Inhalation of a phlai-containing essential oil blend was investigated for acute cognitive effects in healthy adults [PMID:41614069].

Safety & adverse effects

In zebrafish embryos and carp PBMC assays, no cytotoxic effect was observed with cassumunar ginger oil concentrations less than 50 microg/mL [PMID:32019210].

Evidence summary

Evidence is strongest for topical use in musculoskeletal pain (Systematic Review/RCTs). There is emerging clinical evidence (RCTs) for oral use in allergic rhinitis and topical use for diabetic neuropathy and myofascial pain. Preclinical data support broad anti-inflammatory and antimicrobial activities.
