Aloe vera gel promotes healing through multiple mechanisms. Its primary active compound, acemannan (a polysaccharide), stimulates collagen synthesis and fibroblast proliferation, accelerating tissue repair. The gel also contains glycoproteins with anti-inflammatory properties that reduce swelling and pain. Additionally, aloe provides a protective moisture barrier and has mild antimicrobial effects. Evidence Level B supports topical aloe gel for minor burns and wound healing, with studies showing faster epithelialization compared to untreated wounds.
Is aloe vera safe to use during pregnancy?
Aloe vera latex is unsafe during pregnancy and breastfeeding. The anthraquinone compounds (including aloin) stimulate uterine contractions and may cause miscarriage or premature labor. These compounds also pass into breast milk, potentially causing diarrhea in nursing infants. Topical aloe gel applied to intact skin is generally considered safe during pregnancy. Pregnant women should strictly avoid all oral aloe latex products, including laxatives and supplements, and consult their healthcare provider before using any aloe preparations.
Does aloe vera interact with prescription medications?
Yes, aloe vera latex has significant drug interactions. When taken internally, it can cause dangerous potassium loss when combined with diuretics, digoxin, or heart medications, potentially leading to cardiac complications. Aloe latex may also reduce absorption of oral medications due to decreased gut transit time. It should not be combined with other laxatives. Topical aloe gel has minimal systemic absorption and few documented interactions. Always consult a healthcare provider before using aloe latex alongside any prescription medications.
What is the correct dosage of aloe vera for constipation?
For constipation relief, aloe latex containing 100-300mg of aloin is typically taken once daily, usually at bedtime. Effects generally occur within 6-12 hours. Due to risks of electrolyte imbalance and dependence, aloe latex should not be used for more than 7 consecutive days without medical supervision. Evidence Level B supports short-term use only. Topical aloe gel can be applied liberally to affected skin areas as needed. Always follow product-specific labeling instructions.
What are the side effects of taking aloe vera latex?
Aloe vera latex commonly causes abdominal cramping and diarrhea. More serious effects include significant electrolyte loss, particularly potassium depletion, which can lead to muscle weakness and cardiac issues with prolonged use. Chronic use may cause dependence, making normal bowel function difficult without laxatives. Kidney damage has been reported with extended use. People with kidney disease, hemorrhoids, or intestinal obstruction should avoid aloe latex entirely. Evidence Level B supports these safety concerns. Topical gel may cause skin irritation in sensitive individuals.

Summary
Aloe vera is a succulent plant recognized for its diverse health-promoting, skin care, and medicinal properties [PMID:37915246], including anti-inflammatory, antimicrobial, and wound healing activities [PMID:32183224].
Background
Aloe vera (Aloe barbadensis Miller) is a perennial succulent plant belonging to the Lily family [PMID:31643946, PMID:31470079]. It is widely used in cosmetology, pharmaceuticals, and as a dietary supplement [PMID:19882025, PMID:26986231].
Traditional uses
Traditionally used to treat skin injuries such as burns, cuts, insect bites, and eczemas, as well as digestive problems [PMID:32183224]. In Mexico and other countries, it has been used for anti-inflammatory and cosmetic purposes, with the yellow juice used as a laxative and the mucilaginous gel used for arthritis, gout, acne, dermatitis, burns, and peptic ulcers [PMID:27507917].
Active compounds
Aloe vera contains over 200 bioactive compounds [PMID:37915246]. Key constituents include polysaccharides (notably acemannan and acetylated mannans) [PMID:31010204, PMID:10624859], anthraquinone derivatives (aloe-emodin, aloin, emodin) [PMID:32183224, PMID:37915246], aloesin [PMID:32183224], phenolic acids, flavonoids, saponins, alkaloids, and terpenoids [PMID:37915246]. It also contains vitamins C and E, zinc, and amino acids [PMID:34206744].
Mechanism of action
Aloe vera exhibits anti-oxidative properties by stimulating the secretion of superoxide dismutase and peroxidase [PMID:39458379]. It contains a carboxypeptidase that inactivates bradykinin in vitro and substances that inhibit thromboxane formation in vivo [PMID:31643946]. Its polysaccharides provide immunomodulatory effects [PMID:10624859, PMID:28216559].
Clinical evidence
Evidence CSkin Wound Healing
Aloe vera plays an important role in promoting wound healing and is used in wound dressings to accelerate regeneration of damaged skin [PMID:33066720, PMID:31209704].
Evidence CBlood Glucose/Diabetes
Clinical studies demonstrate efficacy in reducing blood glucose levels [PMID:37915246].
Evidence CCancer/Melanoma
Bioactive compounds such as aloin and acemannan show potential antineoplastic activity and can induce apoptosis in cancer cells [PMID:37915246, PMID:36297440].
Evidence BDentistry (Pulpotomy)
In a study of 15 primary molars, Aloe vera gel was used as a healing agent in pulpotomy with no reported pain or mobility after one month [PMID:27507917].
Safety & adverse effects
Ingestion is associated with diarrhea, hypokalemia, pseudomelanosis coli, kidney failure, phototoxicity, and hypersensitive reactions [PMID:26986231]. Oral forms have been linked to rare instances of clinically apparent liver injury [PMID:31643946]. Whole leaf extract has shown evidence of carcinogenic activity in rats and is classified by the IARC as a possible human carcinogen (Group 2B) [PMID:26986231].
Evidence summary
The evidence is primarily based on narrative and systematic reviews (Level C), with some in vitro and in vivo data. While traditional and preliminary clinical use is extensive, particularly for wound healing and glucose reduction, larger controlled trials are needed to determine real efficacy [PMID:19882025, PMID:26090436].
