Modern use is restricted to pharmaceutical santonin preparations due to significant toxicity risks.
Santonin acts as a selective agonist at GABA-gated chloride channels in nematode neurons, causing hyperpolarization and flaccid paralysis of the worm. Artemisin and other sesquiterpene lactones may additionally inhibit acetylcholinesterase and disrupt mitochondrial function in parasites. In the host, santonin exhibits mild cholinergic activity, contributing to side effects such as nausea and xanthopsia at therapeutic doses.
