Question 1

How does capsaicin work to relieve pain?

Accepted Answer

Capsaicin, the active compound in Capsicum frutescens, works by binding to TRPV1 (transient receptor potential vanilloid 1) receptors on sensory nerve endings. This initially causes a burning sensation and release of substance P, a neurotransmitter responsible for pain signaling. With continued application, substance P becomes depleted from nerve terminals, resulting in reduced pain signal transmission. This desensitization effect provides analgesic benefits for neuropathic and musculoskeletal pain conditions (Evidence Level B).

Question 2

Can I take cayenne pepper with blood thinners or anticoagulants?

Accepted Answer

Capsicum frutescens may interact with anticoagulant and antiplatelet medications. Capsaicin can affect platelet aggregation and may increase bleeding risk when combined with drugs like warfarin, aspirin, clopidogrel, or other blood thinners. Additionally, topical capsaicin products may enhance transdermal absorption of other medications applied to the same area. Patients taking anticoagulants should consult a healthcare provider before using concentrated capsaicin supplements (Evidence Level B).

Question 3

What is the recommended dosage for Capsicum frutescens supplements?

Accepted Answer

For oral use as a digestive stimulant, typical doses range from 1-2 grams of dried fruit daily in culinary amounts, or standardized extracts providing 30,000-100,000 Scoville Heat Units (SHU) of capsaicin. For topical pain relief, creams containing 0.025-0.075% capsaicin are applied 3-4 times daily to affected areas. Higher concentrations require medical supervision. Always start with lower doses to assess tolerance, as capsaicin potency varies significantly between preparations (Evidence Level B).

Question 4

Is cayenne pepper safe during pregnancy and breastfeeding?

Accepted Answer

Capsicum frutescens is considered unsafe during pregnancy and should be avoided in medicinal amounts. High doses of capsaicin may stimulate uterine contractions and potentially cause miscarriage or premature labor. While small culinary amounts in food are generally acceptable, concentrated supplements and high-dose topical preparations are not recommended. Breastfeeding mothers should also exercise caution, as capsaicin passes into breast milk and may cause infant irritation or digestive upset (Evidence Level B).

Question 5

What are the side effects of taking capsaicin supplements?

Accepted Answer

Common side effects of Capsicum frutescens include burning sensations in the mouth and throat, gastrointestinal irritation, stomach pain, and diarrhea. Topical applications frequently cause initial burning, stinging, and skin redness at application sites, though this typically diminishes with continued use. People with GERD, peptic ulcers, or sensitive digestive conditions should avoid oral supplements due to potential mucosal irritation. Eye contact with capsaicin causes severe burning and requires thorough flushing (Evidence Level B).
