Cerbera manghas is a highly toxic coastal tree containing cardiac glycosides such as cerberin, neriine, and oleandrin-related compounds. Traditionally used topically for skin diseases and as an arrow poison, it is considered extremely dangerous and not recommended for any medicinal use due to risk of fatal cardiotoxicity.
The primary toxic compounds, cerberin and related cardiac glycosides, inhibit the Na+/K+-ATPase pump in cardiac myocytes, leading to increased intracellular sodium and calcium, which enhances myocardial contractility and can cause arrhythmias. Additionally, these compounds may affect vagal tone and induce bradycardia. At toxic doses, they cause severe cardiac depression, hyperkalemia, and ultimately cardiac arrest.
