Coptis japonica (Coptis japonica): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Coptis japonica work in the body?

Coptis japonica exerts its medicinal effects primarily through isoquinoline alkaloids, specifically berberine, coptisine, palmatine, and worenine. These compounds demonstrate broad-spectrum antimicrobial activity by intercalating into microbial DNA and inhibiting bacterial enzymes. Additionally, berberine modulates inflammatory signaling pathways, such as NF-κB, to reduce cytokine production. This dual mechanism of antimicrobial and anti-inflammatory action supports its traditional use in Kampo medicine for gastrointestinal infections and inflammatory digestive disorders (Evidence Level: C).

Can I take Coptis japonica if I am pregnant or breastfeeding?

No, Coptis japonica is contraindicated during pregnancy and breastfeeding. The herb contains berberine, which can cross the placenta and stimulate uterine contractions, potentially leading to miscarriage or preterm labor. Furthermore, berberine exposure is linked to a significant risk of neonatal jaundice and kernicterus. Due to these safety concerns, pregnant individuals and neonates must strictly avoid this herb (Evidence Level: C).

What is the recommended dosage for Coptis japonica?

The standard adult dosage for Coptis japonica rhizome is typically 3 to 6 grams per day when prepared as a tea or decoction. In traditional Kampo practice, it is often combined with other herbs to moderate its bitter, cold properties. Due to its potency and potential for gastrointestinal irritation, users should adhere to professional guidance and avoid exceeding recommended amounts (Evidence Level: Traditional).

Does Coptis japonica interact with prescription medications?

Yes, Coptis japonica contains berberine, which inhibits cytochrome P450 enzymes (particularly CYP2D6 and CYP3A4). This inhibition can alter the metabolism of various pharmaceutical drugs, potentially increasing their plasma concentrations and side effects. It may also potentiate the effects of antidiabetic medications, causing hypoglycemia. Patients taking prescription medications should consult a healthcare provider before use (Evidence Level: C).

What are the side effects of Coptis japonica?

Common side effects include gastrointestinal upset, such as nausea, stomach pain, and diarrhea, particularly when taken on an empty stomach or in high doses. The herb's bitter constituents can irritate the gastric mucosa. The most serious adverse effect is the risk of neonatal jaundice and brain damage (kernicterus) in infants exposed during pregnancy. It is contraindicated for individuals with spleen deficiency or cold digestive patterns (Evidence Level: C).

Résumé

Coptis japonica is a perennial herb from the Ranunculaceae family used in traditional medicine, characterized by its rich content of benzylisoquinoline alkaloids [PMID:40609932, PMID:39360417].

Contexte

Coptis japonica is a perennial plant of the Ranunculaceae family widely distributed in Japan [PMID:40609932, PMID:29435791]. Its dried rhizome serves as the source for the domestic herbal medicine Coptidis Rhizoma (Oren) [PMID:29435791].

Usages traditionnels

It is a medicinal herb widely used in Asia, known for anti-inflammatory and metabolic regulatory properties [PMID:40469669].

Composés actifs

The plant contains benzylisoquinoline alkaloids (BIAs) [PMID:39360417], specifically berberine [PMID:32013749, PMID:22410351], palmatine, and coptisine [PMID:10552394]. It also contains magnoflorine [PMID:28228365] and water-soluble Rhizoma Coptidis polysaccharides (RCP) [PMID:40609932].

Mécanisme d'action

Coptis japonica Makino extracts (CJME) inhibit the STAT3 signaling pathway to attenuate cancer cachexia [PMID:40469669] and block VEGF-induced cell cycle transition in G1 by decreasing expression of Cyclin D, Cyclin E, Cdk2, and Cdk4 to suppress angiogenesis [PMID:26924430]. Magnoflorine enhances myoblast differentiation via the p38 MAP-kinase and Akt signaling pathways [PMID:28228365]. In gastric cancer cells, it induces apoptosis through caspase3 activation and modulation of BCL2 and BAX expressions [PMID:15934021].

Preuves cliniques

Preuve DCancer Cachexia

CJME significantly decreased mRNA and protein levels of muscle-specific E3 ubiquitin ligases Atrogin-1 and MuRF1 in myotubes and mitigated wasting in mouse models [PMID:40469669]

Preuve DAngiogenesis (Tumor Growth)

CJME inhibited the proliferation, migration, and invasion of VEGF-stimulated HUVECs and suppressed tube formation [PMID:26924430]

Preuve DGastric Cancer (SNU-668 cells)

Root extract induced apoptosis through caspase3 activation [PMID:15934021]

Preuve DMorphine-induced psychological dependence

Methanolic extract (100 mg/kg) inhibited morphine-induced conditioned place preference (CPP) and inhibited c-fos expression in the brain [PMID:12934646]

Preuve DAsthma and Rhinitis

GHX02 (combination including C. japonica) inhibited the increase of total and immune cells in bronchoalveolar lavage fluid and reduced histological lung injury in mouse models [PMID:38608247]

Preuve D

Sécurité et effets indésirables

In a 28-day repeated-dose oral toxicity study of GHX02 (containing C. japonica), the approximate lethal dosage in rats was estimated to be higher than 5000 mg/kg [PMID:31515828]. Some extracts showed mutagenicity in Salmonella typhimurium TA98, though none showed significance in TA102 [PMID:30847756].

Synthèse des preuves

The current evidence consists primarily of in vitro studies and animal models (Level D), focusing on the pharmacological effects of alkaloids and extracts on cancer, muscle atrophy, and respiratory inflammation.
