Drynaria fortunei (Drynaria fortunei): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Drynaria fortunei help heal bones?

Drynaria fortunei promotes bone healing through its flavonoid compounds, particularly naringin and hesperidin. These constituents stimulate osteoblast proliferation and differentiation while inhibiting osteoclast activity, supporting new bone formation. The herb also contains beta-sitosterol, which may help reduce inflammation around fracture sites. In Traditional Chinese Medicine theory, Drynaria strengthens the kidneys, which govern bone health according to TCM principles. Evidence level: C (preclinical studies support these mechanisms, though human clinical trials remain limited).

Does Drynaria fortunei interact with any medications?

Limited research exists on Drynaria fortunei drug interactions. Its flavonoid content, particularly naringin, may potentially affect cytochrome P450 enzymes similarly to grapefruit, which could alter metabolism of certain medications including statins, calcium channel blockers, and some immunosuppressants. Patients taking anticoagulants or antiplatelet medications should exercise caution, as effects on bleeding risk have not been well studied. Always consult a qualified healthcare provider before combining this herb with prescription medications.

What is the recommended dosage for Drynaria fortunei?

The traditional dosage for dried Drynaria fortunei rhizome is 9-15 grams daily, typically prepared as a decoction. Standardized extracts are commonly dosed at approximately 500mg daily. In Traditional Chinese Medicine, this herb is frequently combined with other herbs rather than used alone. Dosage should be adjusted based on individual constitution and the specific condition being treated. Consult a qualified herbalist or healthcare provider for personalized dosing recommendations.

Is Drynaria fortunei safe during pregnancy?

Drynaria fortunei is considered unsafe during pregnancy and should be avoided. In Traditional Chinese Medicine, this herb has properties that may stimulate blood circulation and affect the uterus, potentially posing risks during pregnancy. There is insufficient safety data regarding use while breastfeeding. Women who are pregnant, planning pregnancy, or breastfeeding should not use this herb without explicit guidance from a qualified healthcare provider.

What is Drynaria fortunei used for in Traditional Chinese Medicine?

Drynaria fortunei, known as 'Gu Sui Bu' (literally meaning 'bone-broken supplement'), is primarily used for bone-related conditions including fractures, bone loss, and delayed fracture healing. It is also prescribed for kidney deficiency patterns, which in TCM theory relates to bone strength and hair health. Additional traditional uses include tinnitus and alopecia. Evidence level: C (based primarily on traditional use and preclinical research, with limited human clinical trials available).

Who should not take Drynaria fortunei?

Drynaria fortunei is contraindicated for individuals with yin deficiency heat or constipation, as it may worsen these conditions according to Traditional Chinese Medicine principles. Pregnant and breastfeeding women should avoid this herb. Those taking medications metabolized by cytochrome P450 enzymes should use caution due to potential interactions. While generally well tolerated at recommended doses, individuals with known allergies to ferns should avoid use. Consult a qualified practitioner before starting treatment.

Résumé

Drynaria fortunei (Gu-Sui-Bu) is a traditional Chinese medicinal herb primarily used to enhance bone healing and treat osteoporosis [PMID:12099280, PMID:35863613].

Contexte

Drynaria fortunei, also known as Gu-Sui-Bu, is a fern whose dried rhizome is utilized in traditional Chinese medicine [PMID:35863613, PMID:30298000].

Usages traditionnels

It is widely used in traditional Chinese medicine for treating fractures, osteoporosis, bruising, and common injuries [PMID:35863613, PMID:30298000].

Composés actifs

Active constituents include total flavonoids [PMID:40069718], naringin [PMID:37831321, PMID:25924506], polysaccharides [PMID:32065504], and various phenolic compounds such as caffeic acid 4-O-β-D-glucopyranoside, kaempferol glycosides, neoeriocitrin, and hesperidin [PMID:39204166]. Flavan-3-ols have also been isolated from the rhizomes [PMID:21737107].

Mécanisme d'action

Drynaria fortunei modulates bone metabolism by promoting osteoblast differentiation and proliferation while inhibiting osteoclast activity [PMID:15633809, PMID:25924506]. Specific mechanisms include the regulation of the Rspo1/Lgr4-mediated Wnt/β-catenin signaling pathway [PMID:37831321], the MAPK/HIF-1α pathway [PMID:40069718], and the SIRT1-FOXO3-DEPP1 axis [PMID:41743852]. It also exhibits anti-inflammatory effects by reducing NF-κB expression [PMID:25924506] and regulating the Notch1-NLRP3 inflammasome [PMID:34907823].

Preuves cliniques

Preuve BPostmenopausal Osteoporosis (PMOP) with Hyperlipemia

Drynaria fortunei improved plasma lipid profiles in elderly female patients via regulation of Notch1-NLRP3 inflammasome-mediated inflammation [PMID:34907823]

Preuve AOsteoporosis (Animal Models)

Extracts and flavonoids significantly improved bone mineral density (BMD), trabecular microarchitecture, and biomechanical properties in ovariectomized rats and zebrafish larvae [PMID:41850642, PMID:35863613, PMID:32065504, PMID:25456426]

Preuve DRheumatoid Arthritis (Animal Model)

Synthèse des preuves

The evidence is strongest for the herb's osteoprotective effects in preclinical animal models (Meta-analysis/Systematic Review level A). There is limited clinical evidence (RCT level B) regarding its effect on lipid profiles in PMOP patients. Other effects (anti-arthritic, neuroprotective, anti-obesity) are currently supported by in vitro or animal studies (Level D).
