Its latex contains ingenol mebutate, a diterpene ester that is FDA-approved for the topical treatment of actinic keratosis. Other active compounds include phorbol esters, jolkinol, and pepluanone, which contribute to its keratolytic and antitumor properties.
Ingenol mebutate induces rapid necrosis of dysplastic keratinocytes by activating protein kinase C (PKC) and disrupting mitochondrial function, leading to cell death. It also triggers a robust inflammatory response and immune-mediated clearance of abnormal cells. Phorbol esters may promote tumor growth in some contexts, but ingenol mebutate's unique dual action of initial cell death followed by immune activation underlies its therapeutic effect.
