Gnetum gnemon (Gnetum gnemon): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Gnetum gnemon help lower blood sugar?

Gnetum gnemon contains bioactive compounds including gnetin C, resveratrol, and stilbenes that demonstrate antioxidant activity. These compounds may help protect pancreatic beta cells from oxidative damage and improve insulin sensitivity through free radical scavenging mechanisms. The flavonoids present may also contribute to glucose metabolism modulation. Research suggests these stilbenoids could inhibit carbohydrate-digesting enzymes, potentially slowing glucose absorption in the digestive tract. However, human clinical trials are limited, and evidence remains preliminary (Evidence Level C). More research is needed to fully understand the antidiabetic mechanisms.

Does Gnetum gnemon interact with diabetes medications?

There are no well-documented drug interactions for Gnetum gnemon in scientific literature. However, due to its potential blood sugar-lowering effects, combining it with antidiabetic medications like metformin, sulfonylureas, or insulin could theoretically cause additive effects leading to hypoglycemia. Patients taking diabetes medications should monitor blood glucose levels closely and consult a healthcare provider before adding Gnetum gnemon supplements. Evidence for interactions remains theoretical (Evidence Level C), as formal clinical interaction studies have not been conducted.

What is the recommended dosage of Gnetum gnemon?

Gnetum gnemon seeds are commonly consumed as food in Southeast Asian cuisine without specific dosage restrictions. For medicinal purposes, leaf preparations are typically prepared as tea using approximately 2-3 grams of dried leaf material daily. Standardized extract dosages have not been established in clinical trials. As with any herbal supplement, starting with lower doses is advisable to assess individual tolerance. There is insufficient clinical evidence to determine optimal therapeutic dosing (Evidence Level C). Consult a qualified healthcare practitioner for personalized recommendations.

Is Gnetum gnemon safe to take during pregnancy?

Gnetum gnemon is considered unsafe during pregnancy and should be avoided. While the seeds are consumed as food in some Southeast Asian cultures, medicinal preparations of the leaves have not been evaluated for safety in pregnant women. The resveratrol and other stilbene compounds present may have hormonal or uterine effects that could potentially harm fetal development. Pregnant women should not use Gnetum gnemon supplements. There is insufficient safety data, and the precautionary principle applies (Evidence Level C). Breastfeeding women should also avoid this herb due to lack of safety information.

Can Gnetum gnemon cause gout flare-ups?

Yes, Gnetum gnemon seeds contain purines that may trigger gout flares in susceptible individuals when consumed excessively. People with gout, hyperuricemia, or a history of kidney stones should exercise caution and limit their intake of Gnetum gnemon seeds. The purine content can increase uric acid levels, potentially precipitating painful gout attacks in predisposed individuals. Moderate consumption as food is generally well-tolerated, but those with gout should consult a healthcare provider before using Gnetum gnemon medicinally (Evidence Level C based on traditional use and nutritional analysis).

Résumé

Gnetum gnemon, commonly known as melinjo, is a Southeast Asian gymnosperm plant whose seeds and leaves contain bioactive stilbenoids, including resveratrol and its dimers, which exhibit antioxidant, anti-inflammatory, and metabolic effects [PMID:24602829, PMID:35227186].

Contexte

Gnetum gnemon is an evergreen dioecious gymnosperm native to Southeast Asia, particularly Indonesia [PMID:24602829, PMID:23859249]. It is cultivated as a food source, with its fruits, seeds, and leaves (var. tenerum) used as vegetables [PMID:24602829, PMID:37893741, PMID:39247385].

Usages traditionnels

It is commonly used as a vegetable and medicinal plant among indigenous people in Southeast Asia and North-East India [PMID:39247385, PMID:36606027].

Composés actifs

The seeds are rich in stilbenoid derivatives, including trans-resveratrol, resveratrol dimers such as gnetin C, gnetin L, gnemonoside A, gnemonoside C, and gnemonoside D [PMID:24602829, PMID:19222220, PMID:32152335]. Other compounds include gnetumal (a phenylheptanoid), p-coumaric acid [PMID:32323574], and various flavonoids and phenylethanolamine derivatives [PMID:41634351].

Mécanisme d'action

Melinjo seed extract (MSE) may modulate energy balance by inducing uncoupling protein 1 expression in brown adipose tissue [PMID:30340811]. It has been shown to activate the Sirt1/AMPK and Nrf2/antioxidant defense pathways [PMID:25091897, PMID:36319994] and regulate the AMPK/NF-κB/Sirt1 pathway to attenuate colonic inflammation [PMID:39058737]. Gnetin C may target NAD+ and energy metabolism by downregulating NMNAT2 in prostate cancer cells [PMID:40393106].

Preuves cliniques

Preuve BHyperuricemia

MSE significantly reduced serum uric acid levels and increased HDL cholesterol in healthy adult males [PMID:24454499].

Preuve BObesity/Metabolic Syndrome

In mice, MSE suppressed high-fat diet-induced fat accumulation, hyperglycemia, and hyperinsulinemia [PMID:30340811], and restored obesity-induced sleep architecture changes [PMID:39245578]. In humans, MSE supplementation increased adiponectin multimerization [PMID:32152335].

Preuve DCancer

MSE and gnetin C inhibited proliferation and induced apoptosis in pancreatic, prostate, breast, and colon cancer cell lines in vitro [PMID:26408414]. Gnetin C delayed the progression of preneoplastic lesions in a transgenic prostate cancer mouse model [PMID:36551523].

Preuve DPeriodontitis

Resveratrol dimer-rich melinjo extract caused bone healing and decreased local oxidative damage in a mouse model of experimentally induced periodontitis [PMID:36319994].

Preuve DColitis

Melinjo endosperm extract attenuated colonic inflammation and regulated mucin expression in a mouse colitis model [PMID:39058737].

Sécurité et effets indésirables

MSE powder was well tolerated in humans up to 5000 mg [PMID:24495149]. In rats, the no observed adverse effect level (NOAEL) was determined to be 1000 mg/kg/day, and it tested negative for genotoxicity [PMID:24602829]. Leaf extract showed no significant adverse effects in mice at 3000 mg/kg [PMID:39624614].

Grossesse et allaitement

In rat models, melinjo extract intake during lactation stimulated hepatic AMPK in offspring and attenuated the development of hypertension and hyperglycemia in female offspring of fructose-fed pregnant dams [PMID:27288341, PMID:28925591].

Synthèse des preuves

The evidence consists primarily of in vitro studies and animal models (Level D), with a few small-scale human randomized controlled trials (Level B) focusing on uric acid and adiponectin. Most pharmacological claims are based on murine models.
