Hachimijiogan (Eight-herb Rehmannia formula): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Hachimijiogan work for kidney yang deficiency?

Hachimijiogan works through multiple bioactive compounds including catalpol, rehmannioside, loganin, alisol, and paeoniflorin. These compounds are believed to support adrenal and renal function, improve microcirculation, and modulate glucose metabolism. In Kampo theory, the formula warms and tonifies kidney yang, addressing symptoms like cold extremities, frequent urination, and lower back pain. Research suggests potential effects on insulin sensitivity and renal blood flow, though clinical evidence remains limited (Evidence Level C).

Can Hachimijiogan interact with diabetes medications?

Hachimijiogan may potentially enhance the glucose-lowering effects of antidiabetic medications, including insulin and oral hypoglycemic agents. The formula contains compounds like catalpol and alisol that may affect blood sugar regulation. Patients taking diabetes medications should monitor blood glucose closely and consult a healthcare provider before combining treatments. Dose adjustments may be necessary to prevent hypoglycemia. Evidence for specific interactions is limited (Evidence Level C).

What is the standard dosage for Hachimijiogan?

The standard adult dosage for Hachimijiogan is standardized Kampo extract granules taken as directed by a qualified practitioner, typically 2-3 times daily before or between meals. Traditional granule formulations usually contain approximately 2-7 grams of the eight-herb blend per day. Dosage may vary based on individual constitution, symptom severity, and concurrent conditions. Always follow manufacturer instructions or practitioner guidance, as improper dosing may cause gastrointestinal discomfort (Evidence Level C/Traditional).

Is Hachimijiogan safe to take during pregnancy?

Hachimijiogan is considered unsafe during pregnancy and should be avoided. The formula contains herbs that may stimulate uterine contractions or affect fetal development, particularly at therapeutic doses. Prepared Rehmannia and other components have not been established as safe for pregnant individuals. Women who are pregnant, planning pregnancy, or breastfeeding should consult a qualified healthcare provider before using any Kampo formulas. This contraindication is based on traditional use principles and safety concerns.

What is Hachimijiogan used to treat?

Hachimijiogan is primarily used in Kampo and Traditional Chinese Medicine to address kidney yang deficiency, a pattern characterized by cold extremities, frequent urination, lower back pain, and fatigue. It is one of the most prescribed Kampo formulas in Japan, particularly for elderly patients with deficiency conditions. Research has explored its potential applications in diabetic nephropathy and metabolic disorders. The formula tonifies kidney yang and supports overall vitality in aging individuals (Evidence Level C/Traditional).

What side effects can Hachimijiogan cause?

Common side effects of Hachimijiogan include mild gastrointestinal upset, bloating, and digestive discomfort. These effects are more likely in individuals with cold-deficient constitution or pre-existing digestive weakness. Some patients may experience loose stools or diarrhea, particularly if the formula is contraindicated for their constitution. Side effects are generally mild and may resolve with dosage adjustment. Discontinue use if symptoms persist and consult a qualified Kampo practitioner for constitutional assessment (Evidence Level C/Traditional).

Résumé

Hachimijiogan (also known as Ba-Wei-Di-Huang-Wan) is a traditional Kampo/Chinese medicine formula used to treat various disorders associated with aging, kidney dysfunction, and metabolic issues [PMID:11367538, PMID:19409475].

Contexte

Hachimijiogan is a traditional Japanese (Kampo) and Chinese medicine formula [PMID:28018500, PMID:26719284]. It is composed of eight ingredients: Rehmannia glutinosa root (steamed & dried), Cornus officinalis fructus (dried), Dioscorea oppositifolia root (dried), Alisma plantago-aquatica subsp. orientale tuber (dried), Poria cocos sclerotium (dried), Paeonia x suffruticosa bark (dried), Cinnamomum cassia bark (dried), and Aconitum carmichaeli lateral root (dried & processed) [PMID:26719284].

Usages traditionnels

It has been used for diabetes, urinary frequency treatments [PMID:26719284], male infertility with oligozoospermia [PMID:29259400], and various diseases associated with aging, including osteoporosis [PMID:22543171]. It is also used for patients with 'jinkyo' (kidney hypofunction) [PMID:29755984].

Composés actifs

The formula contains eight herbal ingredients, including Cinnamomi cortex, which has been identified as a component that activates PPARalpha [PMID:18469482].

Mécanisme d'action

Hachimijiogan may activate peroxisome proliferator-activated receptor alpha (PPARalpha) in renal cells, primarily via its Cinnamomi cortex component [PMID:18469482]. It has been shown to inhibit glucagon-induced CREB activation by inhibiting its interaction with CREB-binding protein (CBP) [PMID:32215330]. Other observed effects include the upregulation of myogenin to promote myogenic differentiation [PMID:37779032], shifting T cell differentiation from Th1 to Th2 predominance [PMID:11367538], and increasing the expression of hypoxia-inducible factor-1alpha (HIF-1alpha) in the renal cortex [PMID:21423692].

Preuves cliniques

Preuve BPeripheral Arterial Disease (PAD)

Administration for 6 months significantly improved Walking Impairment Questionnaire (WIQ) scores regarding pain, distance, speed, and total score [PMID:28018500]

Preuve DHyperprolactinemia in infertile women

Treatment (5.0 to 10.0 g/day) reduced serum prolactin levels in 15 out of 18 patients, with 11 patients conceiving and delivering normally [PMID:2633624]

Preuve DBenign Prostatic Hyperplasia

Two-week low-dose administration significantly reduced International Prostate Symptom Score (IPSS) for emptying and weak stream, and improved max flow rate [PMID:14598687]

Preuve B

Sécurité et effets indésirables

One case report identified drug-induced pleuritis (pleural effusion) in a 78-year-old man, which resolved upon discontinuation [PMID:40738677]. In a study of patients with benign prostatic hyperplasia, two patients discontinued use due to epigastric discomfort [PMID:14598687].

Synthèse des preuves

Evidence consists primarily of animal models (rats/mice) and small-scale clinical studies or case reports. While some positive outcomes are noted for PAD and prostatic symptoms, large-scale RCTs are lacking.
