Question 1

How does Centella asiatica work for wound healing?

Accepted Answer

Centella asiatica works by stimulating fibroblast proliferation and increasing collagen synthesis, specifically Type I and III collagen. The active compounds, known as triterpenoids (asiaticoside and madecassoside), enhance the production of extracellular matrix components and promote angiogenesis. This biological mechanism accelerates tissue repair, improves wound tensile strength, and helps modulate the inflammatory response during the healing process (Evidence Level C).

Question 2

Does topical Centella asiatica interact with other medications?

Accepted Answer

There are no significant documented drug interactions for topical Centella asiatica. Because the application is localized to the skin, systemic absorption of active compounds like asiaticoside is minimal, reducing the risk of interactions with oral medications. However, patients using other topical medicated creams or ointments on the same area should consult a healthcare professional to ensure the products are compatible and do not cause irritation when combined (Evidence Level C).

Question 3

How often should I apply Centella asiatica cream?

Accepted Answer

The standard dosage for topical Centella asiatica involves applying a cream or ointment containing 1% to 2% asiaticoside concentration to the affected area. For best results in wound healing and scar reduction, it should be applied two to three times daily. Users should cover the area with a thin layer and continue use until the wound has healed or the scar appearance has improved, following specific product guidelines (Evidence Level C).

Question 4

Is Centella asiatica safe to use during pregnancy?

Accepted Answer

Topical Centella asiatica is generally considered safe for use during pregnancy. Due to minimal systemic absorption, it is frequently recommended by dermatologists for managing stretch marks and promoting skin elasticity. While oral forms require more caution, topical applications pose little risk to fetal development. As with any skincare product during pregnancy, it is advisable to consult a healthcare provider prior to use (Evidence Level C).

Question 5

Can Centella asiatica cream remove old scars?

Accepted Answer

Centella asiatica is effective in improving the appearance of both new and existing scars. It helps regulate collagen production and prevents the over-accumulation of fibrous tissue, which can flatten and soften hypertrophic scars and keloids. While it may not completely 'remove' old scars, regular application can significantly improve texture, color, and elasticity. It is a staple in dermatology and Korean skincare for scar management (Evidence Level C).
