Iberis Amara Herb (Iberis amara): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Iberis amara work as a digestive bitter?

Iberis amara contains bitter compounds, particularly cucurbitacins and glucosinolates like glucoiberin, which stimulate bitter taste receptors on the tongue and throughout the digestive tract. This stimulation triggers the release of digestive hormones, increases gastric acid secretion, enhances bile flow, and promotes pancreatic enzyme production. The herb also contains flavonoids such as kaempferol and quercetin that provide anti-inflammatory effects. Evidence Level: C.

Does Iberis amara interact with any medications?

Iberis amara may interact with medications metabolized by the liver due to its potential hepatotoxic effects from cucurbitacin compounds. Patients taking hepatotoxic medications, including acetaminophen and certain antibiotics, should avoid combining them with this herb. Additionally, because Iberis amara stimulates gastric acid secretion, it may reduce the effectiveness of proton pump inhibitors and H2 blockers. Consult a healthcare provider before use with any prescription medications. Evidence Level: C.

What is the recommended dosage of Iberis amara?

The clinically studied dosage of Iberis amara is approximately 20mg of liquid extract, typically administered as part of the fixed combination product Iberogast. This proprietary formulation has been investigated for functional dyspepsia and irritable bowel syndrome in adults. Crude extracts or raw herb preparations are not recommended due to cucurbitacin toxicity risks. Always follow product labeling or consult a qualified healthcare practitioner for individualized dosing guidance. Evidence Level: C.

Is Iberis amara safe during pregnancy and breastfeeding?

Iberis amara is contraindicated during pregnancy and breastfeeding. The herb contains cucurbitacin compounds that may cause uterine stimulation and pose risks of fetal harm. Additionally, the potential for hepatotoxicity presents serious safety concerns during pregnancy. Women who are pregnant, planning to become pregnant, or breastfeeding should avoid this herb entirely. Evidence Level: C.

What are the side effects of Iberis amara?

Iberis amara may cause hepatotoxicity, particularly when crude extracts are used, due to its cucurbitacin content. Other potential side effects include gastrointestinal discomfort, nausea, and allergic reactions in sensitive individuals. The herb is contraindicated in patients with pre-existing liver disease and should not be given to children due to toxicity concerns. Standardized commercial formulations may reduce these risks compared to raw herb preparations. Evidence Level: C.

What is Iberis amara used for in herbal medicine?

Iberis amara, commonly known as bitter candytuft, is primarily used as a digestive bitter for treating functional dyspepsia and irritable bowel syndrome. In European folk medicine, it was historically employed as a cardiac and respiratory tonic. Modern clinical use focuses on its role in combination formulas for gastrointestinal disorders, where its bitter compounds stimulate digestive secretions and promote healthy gut motility. Evidence Level: C.

Résumé

Iberis amara (bitter candytuft) is a European medicinal plant used primarily for treating functional gastrointestinal disorders [PMID:12618547, PMID:12625311].

Contexte

Iberis amara L. (Brassicaceae) is widespread in Europe, typically growing in warm, sunny, dry, loamy, and loessial soils high in calcium [PMID:12618547].

Usages traditionnels

Bitter herbal drugs have been used since ancient times to treat patients with dyspeptic symptoms [PMID:11694755].

Composés actifs

The plant contains amines, cucurbitacines (including cucurbitacins J and K), flavonoglycosides (such as kaempferol-3,4'-di-O-beta-glucopyranoside-7-O-alpha-rhamnopyranoside), and mustard oil glycosides (including glucoiberin and sinigrin) [PMID:12618547, PMID:19320289, PMID:14117728, PMID:24249231, PMID:11975492]. It also contains sinapic acid esters [PMID:10925016].

Mécanisme d'action

Fresh plant extract exhibits a tonicising effect on the smooth muscles of the stomach and small intestine [PMID:12618547, PMID:12618546]. It has demonstrated antiulcerogenic effects by reducing stomach acid release and leucotriene concentration while increasing prostaglandin E2 content [PMID:12618547]. Additionally, it possesses the potential to scavenge reactive oxygen species (ROS), particularly those generated by myeloperoxidase (HOCl) and peroxynitrite [PMID:13677247].

Preuves cliniques

Preuve AFunctional Dyspepsia

A systematic review of three placebo-controlled trials and one reference-controlled trial (595 patients), plus one observational study (2267 patients), indicated that the herbal combination Iberogast (containing Iberis amara) significantly reduced gastrointestinal symptom scores [PMID:15927925].

Preuve DInflammation (Animal Model)

In rat models of acute (carrageenan paw edema) and chronic (adjuvant-induced arthritis) inflammation, Iberis amara extract dose-dependently reduced edema and decreased mediators including TNF-α, IL-1β, and prostaglandin E2 [PMID:26287692].

Preuve DColon Carcinoma (In Vitro/Animal Model)

Hydroethanolic extract inhibited proliferation of HT-29 and T84 colon carcinoma cells via G2/M phase cell cycle arrest and ROS-induced apoptosis; oral gavage in rats resulted in significant inhibition of tumor growth [PMID:27050665].

Sécurité et effets indésirables

In the context of the combination product Iberogast, the preparation was found to reduce symptoms of dyspepsia without causing central nervous side effects [PMID:15927925].

Synthèse des preuves

Evidence for Iberis amara is primarily based on its inclusion in the multi-component preparation Iberogast, supported by systematic reviews of clinical trials for dyspepsia (Level A). Independent pharmacological effects (anti-inflammatory, anti-ulcer, and anti-cancer) have been demonstrated in in vitro and animal models (Level D).
