Phragmites australis, a cosmopolitan wetland grass, is utilized in Traditional Chinese Medicine for its heat-clearing, antiemetic, and diuretic properties. Modern research suggests antipyretic, antiemetic, and diuretic effects, attributed to active compounds such as coixol, triterpenoids, flavonoids, and polyphenols. Evidence for these uses is limited (Level C), primarily based on traditional experience and preliminary studies.
Coixol, a benzoxazinoid, may exert antiemetic effects via antagonism of serotonin 5-HT3 receptors and modulation of the chemoreceptor trigger zone. Flavonoids and polyphenols contribute to anti-inflammatory and antioxidant actions through inhibition of cyclooxygenase (COX) and lipoxygenase (LOX) pathways, as well as scavenging reactive oxygen species. The diuretic effect is likely mediated by increased renal blood flow and inhibition of sodium-potassium-chloride cotransport in the loop of Henle, similar to other plant-based diuretics.
