Elderberry Syrup (Sambucus nigra (standardized)): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does elderberry syrup work to fight the flu?

Elderberry syrup works through multiple mechanisms. The anthocyanins and flavonoids in standardized elderberry extracts bind to influenza virus particles, preventing them from entering and infecting host cells. These compounds also stimulate immune system activity by increasing cytokine production. Additionally, elderberry's antioxidant properties help reduce oxidative stress during infection. Evidence Level B supports these antiviral and immunomodulatory effects, making standardized preparations more reliable than raw elderberry products.

Can I take elderberry syrup with immunosuppressant medications?

Elderberry syrup should not be taken with immunosuppressant medications. Because elderberry stimulates immune system activity, it can potentially reduce the effectiveness of drugs designed to suppress immune function, including corticosteroids, cyclosporine, and other immunosuppressants prescribed for autoimmune conditions or organ transplant patients. Individuals with autoimmune conditions should avoid elderberry products entirely. Always consult your healthcare provider before combining elderberry with any prescription medications.

What is the correct dosage of elderberry syrup for adults?

For adults, the standard dosage of standardized elderberry syrup is 15ml (1 tablespoon) taken four times daily during active illness, such as cold or flu symptoms. For prevention purposes, 15ml once daily is typically recommended. These dosages apply to standardized commercial preparations. Dosing may vary between products, so always follow specific product labels. Evidence Level B supports these dosage ranges based on clinical research in standardized extracts.

Is elderberry syrup safe during pregnancy or breastfeeding?

Elderberry syrup is considered unsafe during pregnancy and breastfeeding due to insufficient safety data. While standardized commercial preparations are processed to remove harmful compounds found in raw elderberries, the lack of pregnancy-specific safety studies warrants caution. Additionally, elderberry's immune-stimulating properties may not be appropriate during pregnancy. Pregnant and breastfeeding women should consult their healthcare provider before using any elderberry products and consider alternative remedies with established safety profiles.

Does elderberry syrup actually help with colds and flu?

Clinical research suggests standardized elderberry syrup can help reduce the duration and severity of cold and flu symptoms. Studies on standardized preparations indicate elderberry may shorten flu duration when taken within the first 48 hours of symptom onset. Evidence Level B supports elderberry's effectiveness for influenza treatment and symptom management. However, elderberry should not replace conventional medical treatment for serious infections. Individuals with persistent or severe symptoms should consult a healthcare provider.

Résumé

Elderberry (Sambucus nigra L.) is used as a natural remedy for viral respiratory infections, demonstrating potential to reduce the duration of influenza symptoms [PMID:15080016, PMID:11399518].

Contexte

Elderberry (Sambucus nigra L.) is a widespread deciduous shrub whose fruits are used in dietary supplements and the food industry [PMID:38998923].

Usages traditionnels

It has been used in folk medicine for centuries to treat influenza, colds, and sinusitis [PMID:15080016], and has a long ethnobotanical history across many cultures as a treatment for viral infections [PMID:28198157].

Composés actifs

Major components include phenolic compounds such as anthocyanins (notably cyanidin-3-glucoside), flavonoids (including rutin), simple phenolic acids, and tannins [PMID:38998923, PMID:35118699, PMID:34505534].

Mécanisme d'action

Elderberry may exert a direct antiviral effect, stimulate the host immune system by inducing dendritic cell maturation and T cell responses [PMID:35409309], and increase the production of inflammatory cytokines [PMID:12455180]. One theory suggests it renders viruses nonfunctional by staining and coating them [PMID:11366542].

Preuves cliniques

Preuve BInfluenza A and B

Oral elderberry syrup (15 ml four times daily for 5 days) relieved symptoms on average 4 days earlier and significantly reduced the use of rescue medication compared to placebo [PMID:15080016].

Preuve BInfluenza

A standardized extract (Sambucol) reduced the duration of flu symptoms to 3-4 days and was associated with higher antibody levels to the influenza virus in the convalescent phase [PMID:11399518].

Preuve BDry Eye Disease (DED)

Combined supplementation of 100 mg elderberry extract with lutein and zeaxanthin for 20 days significantly improved Ocular Surface Disease Index (OSDI) scores [PMID:39770987].

Sécurité et effets indésirables

Elderberry is generally recognized as safe by the US FDA [PMID:28198157].

Synthèse des preuves

Evidence for influenza treatment is based on small clinical trials (Level B) and in vitro studies, while other metabolic and immune effects are primarily demonstrated in animal models or in vitro (Level D).
