Carissa macrocarpa (Carissa macrocarpa): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does Carissa macrocarpa work as a cardiac tonic?

Carissa macrocarpa contains trace amounts of cardiac glycosides and compounds like carissone. These constituents are believed to influence heart function by potentially strengthening cardiac contractions, though this is based on phytochemical analysis rather than clinical trials (Evidence Level C). The presence of flavonoids and tannins contributes to antimicrobial properties, supporting its traditional use for infections. The glycosides likely inhibit sodium-potassium ATPase, a mechanism similar to pharmaceutical cardiac drugs, but rigorous human studies are lacking.

Can I take Carissa macrocarpa with heart medication?

Caution is strongly advised when combining Carissa macrocarpa with heart medications. Because the plant contains trace cardiac glycosides, taking it alongside pharmaceutical heart drugs like digoxin could potentiate effects, leading to arrhythmias or toxicity (Evidence Level C/D). It may also interact with blood pressure medications. Always consult a qualified healthcare provider before combining herbal remedies with prescription cardiac drugs to avoid adverse interactions.

What is the recommended dosage for Carissa macrocarpa bark?

Traditional usage suggests a bark decoction of 5 to 10 grams taken twice daily (Evidence Level C/Traditional). This preparation is typically used for its cardiac tonic properties or to address infections. The fruit is consumed freely as food. However, due to the presence of cardiac glycosides, precise dosing is critical to avoid toxicity. Self-medication is discouraged without professional guidance, especially regarding root preparations which have higher concentrations of active compounds.

Is Carissa macrocarpa safe during pregnancy?

No, Carissa macrocarpa is considered unsafe during pregnancy. The roots and bark contain cardiac glycosides and other uterine stimulants that may cause miscarriage or harm fetal development (Evidence Level C/Traditional). While the ripe fruit is generally considered edible and nutritious, medicinal preparations using the bark or root should be strictly avoided by pregnant or breastfeeding women. Always err on the side of caution and consult a medical professional.

What are the traditional uses of Carissa macrocarpa?

In Zulu traditional medicine, Carissa macrocarpa is primarily used as a cardiac tonic and to treat various infections. The bark and roots are prepared to address heart conditions, while the plant's antimicrobial properties make it useful for skin conditions and wound healing (Evidence Level C). The fruit, known as the Natal plum, is a nutritious food source rich in Vitamin C. Despite these traditional applications, scientific validation remains limited.

Contexte

Carissa macrocarpa (Apocynaceae) is a woody shrub found in subtropical savannas of South Africa and has also been recorded in Tunisia [PMID:42256167, PMID:33473704, PMID:39942880].

Usages traditionnels

The fruits are widely consumed in African countries for the preparation of traditional foodstuff and for their beneficial health effects [PMID:30478775].

Composés actifs

The plant contains anthocyanins, specifically cyanidin 3-O-β-sambubioside and cyanidin 3-O-glucoside [PMID:36076736, PMID:34207411]. Other identified compounds include flavonoid glycosides (derivatives of kaempferol, quercetin, and isorhamnetin), ellagic acid, catechin, epicatechin, apigenin, luteolin, and various essential oils such as pentadecanal and (E)-nerolidol [PMID:42114831, PMID:34207411, PMID:32260360, PMID:29907990].

Mécanisme d'action

Leaf extracts have demonstrated the ability to modulate p53 expression in colorectal cancer cell lines [PMID:42114831]. Flavan-3-ol derivatives from the leaves show strong binding affinity with TACE enzymes, contributing to neuroprotective activity [PMID:34959705]. Additionally, extracts have shown the ability to inhibit acetylcholinesterase and tyrosinase [PMID:41986532].
