Racine d'Iboga congolaise (Tabernanthe iboga): Benefits, Dosage, Safety & Evidence

Questions fréquentes

How does iboga root work for addiction treatment?

Iboga root contains ibogaine, which acts on multiple neurotransmitter systems including opioid, serotonin, and NMDA receptors. Research suggests ibogaine may interrupt addiction patterns by modulating neuroplasticity and reducing withdrawal symptoms. The mechanism involves ibogaine's conversion to noribogaine, which has prolonged effects on mood and craving pathways. Evidence level: C. However, the exact mechanism remains incompletely understood and requires further study.

What drugs interact with iboga root and ibogaine?

Iboga root has serious drug interactions, particularly with medications affecting heart rhythm or serotonin levels. Combining ibogaine with SSRIs, MAOIs, or other antidepressants can cause serotonin syndrome. Concurrent use with opioids, stimulants, or medications that prolong the QT interval significantly increases cardiac arrhythmia risk. Due to these dangerous interactions, ibogaine treatment requires thorough medical screening and adequate drug-free periods before administration. Evidence level: C.

What is a safe dosage of iboga root?

There is no established safe self-administration dosage for iboga root. Traditional Bwiti ceremonies use varying amounts, while medical settings typically administer ibogaine hydrochloride at 10-20 mg/kg for addiction treatment. Dosage must only be determined by qualified medical professionals due to significant cardiac risks. The therapeutic window is narrow, and improper dosing has resulted in fatalities. Evidence level: C. Never attempt self-administration under any circumstances.

Is iboga root safe during pregnancy?

No, iboga root is unsafe during pregnancy. Ibogaine crosses the placenta and may cause fetal harm, including developmental abnormalities and miscarriage risk. The uterine-stimulating properties can trigger premature labor. Additionally, the cardiovascular effects pose serious risks to both mother and fetus. Women of childbearing age should use effective contraception and confirm they are not pregnant before any iboga exposure. Evidence level: C. Avoid completely during pregnancy and breastfeeding.

What is iboga root used for traditionally?

Iboga root is traditionally used in Central African Bwiti initiation ceremonies for spiritual healing and cultural rites of passage. Modern interest focuses on its potential for treating substance addiction, particularly opioid and alcohol dependence. Some seek it for psychotherapeutic insights or spiritual exploration. Research evidence (Level C) suggests possible anti-addictive properties, but ibogaine remains unapproved for medical use in most countries due to significant safety concerns.

What are the dangers and side effects of iboga root?

Iboga root carries serious risks including potentially fatal cardiac arrhythmias, particularly in those with pre-existing heart conditions. Neurological effects include prolonged hallucinations, ataxia, and psychological distress. Deaths have occurred from cardiac complications, often in unsupervised settings. Contraindications include heart disease, psychiatric conditions, liver impairment, and pregnancy. Due to these dangers, ibogaine should only be administered under intensive medical monitoring with emergency cardiac equipment available. Evidence level: C.

Résumé

Tabernanthe iboga is a West African shrub containing ibogaine, an alkaloid investigated for its potential to treat substance use disorders [PMID:35012793, PMID:30216039].

Contexte

Tabernanthe iboga is an African rainforest shrub belonging to the Apocynaceae family [PMID:26807959, PMID:18024037]. It is native to western equatorial Africa and is considered a sacred plant by the people of Gabon [PMID:30030374, PMID:28194562].

Usages traditionnels

The plant has been used for centuries by the Bwiti and Mbiri tribes to induce hallucinations during religious ceremonies [PMID:30216039]. Root bark preparations are used in Central and West African traditional medicine to combat fatigue, as a neuro-stimulant in rituals, and for the treatment of diabetes [PMID:30582133, PMID:22751004].

Composés actifs

The predominant alkaloid is ibogaine [PMID:11942686, PMID:30618175]. Other identified alkaloids include noribogaine (the primary metabolite), coronaridine, ibogamine, and voacangine [PMID:26807959, PMID:30618175].

Mécanisme d'action

Ibogaine and noribogaine inhibit the transport function of the vesicular monoamine transporter 2 (VMAT2) [PMID:41452009]. Its anti-opiate effects may be linked to actions at nicotinic acetylcholine receptor subtypes [PMID:30216039]. In rat models, ibogaine induces enzymes of glycolysis and the tricarboxylic acid (TCA) cycle [PMID:17054944]. It also inhibits acetylcholinesterase, though this effect is considered physiologically negligible [PMID:22200647].

Preuves cliniques

Preuve ASubstance Use Disorders (SUDs)

A systematic review of 24 studies (including 705 individuals) examined the efficacy of ibogaine and noribogaine for SUDs [PMID:35012793].

Preuve COpioid, Cocaine, and Alcohol Addiction

Ibogaine has been proposed as a drug cessation treatment with activities targeting opioids, cocaine, and alcohol [PMID:28194562]. Animal models show it can blunt self-administration of opiates, cocaine, amphetamines, and nicotine [PMID:30216039, PMID:40895173].

Preuve DHyperglycemia (Mice)

Water extract of iboga root bark was studied for potential alleviating effects on high-fat diet-induced hyperglycemia in C57BL/6J mice [PMID:30582133].

Sécurité et effets indésirables

Ibogaine is associated with cardiotoxicity, including QT prolongation, torsades de pointes, and fatalities [PMID:25636206, PMID:30216039]. At micromolar levels, it is both neurotoxic and cardiotoxic [PMID:30216039]. It also possesses hallucinogenic potential [PMID:40895173].

Interactions médicamenteuses

Ibogaine enhances the increase in accumbal dopamine produced by the acute administration of stimulant drugs and potentiates stimulant-induced motor behaviors [PMID:10936624].

Synthèse des preuves

Evidence consists primarily of systematic reviews of case series, preclinical animal models, and phytochemical analyses; high-level clinical trial data is limited.
