Licorice may worsen potassium loss caused by thiazide diuretics, potentially leading to dangerous hypokalemia, cardiac arrhythmias, and muscle weakness.
Glycyrrhizin inhibits 11-beta-hydroxysteroid dehydrogenase, causing cortisol to activate mineralocorticoid receptors, promoting potassium excretion. This adds to diuretic potassium wasting.
